What is a Freedom of Information Request?
The Freedom of Information Act (2000) aims to ensure that public bodies, including general practice contractors who
provide general or personal medical services, are open to the public concerning the way we operate. When you ask
us to provide certain types of information, this is known as a Freedom of Information Request
What Information is routinely published?
Victoria Practice operates under a contract with the NHS and to comply with the requirements of the Act, we have
developed a Publication Scheme, which details all the information that we routinely make available. A copy of the

Who can request information?
The Act, allows any individual to make a request to us for information. We will inform you in writing if we hold
the information required and if we can make it available to you.
How do I make a request?
To make a request you should write to us at the address on this website stating your full name, a postal address
for correspondence and a description of the information required. We accept requests by e-mail and fax as being ‘in writing’.
Please mark your request for the attention of the Practice Manager.
What will the practice do?
Following a request we will reply within 20 working days to tell you if the information is available or not. We will then
try to provide the information in the format you have asked for. We may direct you to a website or provide you with a printed
copy. If we cannot make the information available immediately, we will tell you when we expect to be able to release it.
Do I have to pay for information?
Unless we state otherwise, one copy of anything listed in the Publication Scheme is available free of charge in paper-format
or if it is downloaded from our website or sent electronically.
Is there any information that the practice will not provide?
We will make every effort to provide you with a copy of the information requested and we may have to ask you some questions to
make sure that we provide you with what you need. There may be some documentation or certain parts of that documentation which
we are not allowed to release. Some examples of information we would not be able to release are:
- Someone else’s medical records (protected by the Data Protection Act 1998)
- Personal information (protected by the Data Protection Act 1998)
- A company’s trade secrets.
- Information held in Court records
